The challenge

The owner had been boosting Instagram posts for about eighteen months at roughly $900 a month. Engagement looked healthy — comments, saves, follower growth — but nobody could connect any of it to a booked appointment.

There was no pixel on the booking system, so no conversion data existed at all. The only measure of success was a gut feeling that busy months followed heavy posting.

Creative was whatever had performed organically, which skewed heavily toward before-and-after photos that ran into Meta's advertising policies for health and beauty claims.

    Installed real tracking

    Meta Pixel plus the Conversions API wired into the booking platform, so consultation bookings were attributed server-side even where browser tracking was blocked.

    Separated cold and warm audiences

    Prospecting, engagement retargeting and abandoned-booking retargeting split into distinct campaigns with their own budgets, so spend could be judged and moved on merit.

    Built compliant creative

    Three angles tested against one seasonal offer — practitioner-led explainer video, treatment education carousel and a straightforward offer card — all written within Meta's health advertising policies.

    Tightened the service radius

    Targeting reduced from a 40-mile radius to 12 miles, matching where clients realistically travelled from. Impressions dropped, booked consults rose.

The outcome

Cost per booked consultation fell from $104 to $44 across the first 90 days and held steady through month six.

Monthly consultations grew from 11 to 35 with no increase in ad budget, driven by better targeting and the retargeting layer that previously did not exist.

With attribution finally in place, the practice could see a 6.1× return on ad spend and made the decision to increase budget in month seven from an informed position.
